Conduct ASQ Screenings for SR and Non-SR Children/Warm Line/Inclusion Services


Children birth to five who participate in the School Readiness program receive annual developmental screenings called Ages & Stages Questionnaire (ASQ). The purpose of the developmental screening is to capture a snapshot of a child’s development at a single point in time. Screening identifies children who may not meet the milestone appropriate for their age and might benefit from further assessment and follow-up. When screenings occur regularly, they provide information for tracking a child’s developmental progress.

To complete an ASQ screening for an SR child, go to:

The ASQ Parent Results Review Form must be shared and signed by the parent and uploaded to Providers Services Portal under Document Library, file: ASQ Results-Parent Review form.

To complete an ASQ Screening for a non-SR child, go to:

Before a provider completes an ASQ for a non-SR child, the parent consent form must be completed and uploaded to the Provider Portal – Provider Document Library, under Development Screenings. Please label the document Consent Form for Non SR Child. Contact Liliana Scalf at 904-208-2044, ext. 295 once the Consent Form for Non SR Child has been uploaded.

Inclusion Services


Inclusion supports the right that all children can participate in early learning and care settings alongside their same-age peers regardless of their diverse abilities. The ELC of Duval’s Inclusion Specialists works with families and providers to ensure the child is in the most appropriate environment.

The Americans with Disabilities Act (ADA) became law in 1990. The ADA is a civil rights law that prohibits discrimination against individuals with disabilities in all areas of public life, including jobs, schools, transportation, and all public and private places open to the general public.

Children benefit from

  • Are provided with role models
  • Learn new skills
  • Encouraged to be resourceful and creative

  • Grow professionally by developing new skills

  • Build strong relationships with parents
  • Enhance their credibility as quality providers
